On May 31, 2017, a grain dust explosion at a corn milling facility in Wisconsin resulted in the deaths of five workers and injured at least twelve others. The explosion was caused by an accumulation of combustible corn dust due to insufficient housekeeping and maintenance.

The Occupational Safety and Health Administration (OSHA) investigated the incident and cited the employer for several willful violations. OSHA also referred the case for criminal prosecution, highlighting gross neglect in managing dust control and preventing explosions.
