OhioHealth Grant Medical Center offers a diabetes program aimed at empowering patients through education, personalized care, and ongoing support. The program is led by experts like Rachel Sayre, BSN, RN, CDCES, who is a nurse educator and certified diabetes care specialist.
“Some patients are told over the phone that they have diabetes,” Sayre explains. Her team strives to ease the daunting process of learning about diabetes after such news.
The program emphasizes not only initial patient education but also regular follow-ups. It covers essential topics including understanding diabetes, its effects on the body, medication management, potential complications, and recommended lifestyle changes. Patients receive personal guidance from both a registered nurse educator and a dietitian. The program includes ten hours of education with resources provided in a binder for future reference. Weekend sessions are available as well.
To participate in the classes, patients need a referral from their primary care provider or endocrinologist. However, the office encourages calls for inquiries about referral options or further information on the program.
“We can help go about that process. We can help them place the order. We can help the patient learn more about the program and we can provide the options for the schedules,” says Sayre.
